The forms published by In-Forms are copyrighted publications. All sales are final. We make samples available at no cost for health care providers to evaluate prior to purchase. ( sample request form.)
However, we recognize that errors can be made in either the order by the client or the filling or shipping of an order by In-Forms.
If you receive a wrong form, we will replace the pads on an exchange basis. If you incorrectly ordered a form or forms that you need to exchange, we will charge a re-stocking fee for each pad returned. Depending on the quantities of the original order, you may forfeit any volume discount because of a return.
If In-Forms incorrectly shipped you a form or forms instead of the forms you ordered, we will ship replacement(s) as quickly as we can after notification of the error. Return of the incorrect form(s) will be arranged on a case-by-case basis.
